Handicap Parking Permits

Any person who has a severe visual, audio, or physical impairment,
including partial paralysis, lower limb amputation, chronic heart condition,
emphysema, arthritis, rheumatism, or other debilitating condition which limits
or impairs one’s mobility or ability to walk may be issued a handicap placard.
In addition, any agency or organization, which transports handicapped
persons, may apply for a placard for each vehicle used in that capacity.
The following documents & information are required for issuance
of a permit to an individual:

Section
1 of the Application for Disabled Persons Special Parking Permit must be completed & signed
by the applicant. The applicant’s
signature must be notarized. If
notarized by County Clerk staff, identification must be presented.

If
applicant currently has a handicap plate or a veterans handicap plate
only Section 1 needs to be completed.
The physician’s signature would not be required.
Applicants with handicap plates are only allowed 1 handicap placard.
¨
If
applicant is obviously handicapped, the applicant would complete Section 1 and
the County Clerk could complete Section 2.
The physician’s signature would not be required.
¨
Applicants
that currently do not have a handicap plate or are not obviously handicapped
must complete Section 1 and have physician complete Section 3. The physician
should indicate whether the handicap is temporary or permanent.
The following documents &
information are required for issuance of a permit to an agency or organization:

authorized agent of organization or agency.
The signature must be notarized.
If notarized by County Clerk staff, identification must be presented.

A
statement from the director of the agency or organization verifying the need for
the handicap parking placard and the number of vehicles used in the
transportation of handicapped persons. This
must be on agency letterhead.

Note-If
applicant currently has a handicap parking plate they are not eligible for a
second placard.
